Procedure Forms

Asbestos Manifest 

This form is required for all loads of asbestos that is brought out to the landfill. The driver must have it in his/her possession when transporting the material per EGLE regulations. Please see asbestos page for asbestos policy

Hydrocarbon Contaminated Material (Contaminated Soil) Manifest

This form is required for all hydrocarbon contaminated material (contaminated soil) that is brought to the landfill. The driver must have it in his/her possession when transporting the material. For more information see our hydrocarbon contaminated material (contaminated soil) page.

Scrap Tire Manifest

Businesses who want to haul more than 10 tires at a time can contact the Marquette EGLE at (906) 228-4853 to obtain a Scrap Tire Hauler Registration. The scrap tire manifest form must be filled out for all businesses that are hauling more than 10 scrap tires at a time. Waste Characterization / Demo Form 

This form needs to be submitted with any special wastes such as contaminated soil, demolitions, etc. The following must be included with this form:

  1. A list of all known contaminants within one mile of the waste evacuation. If there are none, it must be clearly stated. 
  2. A complete list of all known testing that has taken place within one mile of the waste materials original location. 
  3. A description of the waste. Ex: “Fuel oil from an auto accident in topsoil”
